Intake forms
Intake forms collect booking details from customers at checkout — contact info, project specifics, preferences. Unlike documents, nothing is signed: the answers are stored on the booking itself.
Form library
Open Dashboard → Forms (Owner or Manager role) to see your form library. Build a form once, then attach it to every booking, to selected services, to add-ons, or both.
Choose New form and set:
- Form name — for your own reference, for example "New client details".
- When should clients complete this? — Every booking, or Selected services and add-ons. With the second option, tick the services and add-ons that should show the form.
Questions
Add questions of these types:
- Short text and Long text — free-form answers.
- Number — numeric answers.
- Single select and Multi select — choose from options you define.
- Yes / no — a simple binary answer.
Each question can be marked required, and you can require the whole form for booking — customers must answer before they can check out.
Where answers appear
Answers land on the booking's detail page under Intake answers. If a booking has no intake form, it shows "No intake answers were captured."
Changes and deletions
Forms are snapshotted onto a booking when it's created. If you edit a form later, the change applies to future bookings only — existing bookings keep the answers and questions they were captured with. Deleting a form removes it from future bookings the same way.
